COTA Tasmania’s Care Finder programme provides free, personalised support for eligible older people who need extra help understanding and accessing aged-care services.

Care Finders can help you understand your options, arrange assessments, complete forms, communicate with service providers and connect with aged-care and other community supports.

The service is designed for older people who do not have family or friends available to help them, or who find aged-care services difficult to understand, access or engage with.

Support is available throughout Tasmania, including rural and remote communities, and may be provided by phone, email, post, home visits or face-to-face meetings.

The Care Finder program is delivered by COTA Tasmania and funded by the Australian Government through Primary Health Tasmania (Tasmania PHN).
