Provides up to 26 weeks of one-on-one support for adults (18+) experiencing severe mental health challenges, even without a formal diagnosis, focusing on building skills, resilience, and personal recovery goals.

Outreach support is available at home or in the community to help manage and prevent future episodes. Contact the Program Coordinator or ask a GP or health professional for referral (eligibility applies).
